To request a transcript, please follow the directions below:
- Download the Transcript Request Form (PDF).
- Fill out the form, including your signature.
- The completed form may be faxed, mailed, or emailed to us.
Important Information:
- Please sign the form, otherwise the request cannot be completed.
- There is no fee for transcript requests.
- Transcripts are processed weekly. Deadline is 4:30 pm on Wednesdays.
